Patient Bill of Rights

AvataCore LLC ("TETRA") is committed to protecting the rights of every patient who uses our telehealth platform. This Bill of Rights describes what you are entitled to expect.
Your Rights
- To be treated with respect. You have the right to considerate, respectful care from all personnel, free from discrimination based on race, ethnicity, national origin, religion, sex, age, disability, sexual orientation, or gender identity.
- To informed consent. You have the right to receive information about your treatment options, including risks, benefits, and reasonable alternatives, in terms you can understand, before agreeing to treatment.
- To refuse treatment. You have the right to decline any recommended treatment or medication, and to be informed of the medical consequences of that decision.
- To know your clinician. You have the right to know the name and credentials of the licensed clinician reviewing your case, and to confirm their licensure in your state.
- To privacy and confidentiality. You have the right to have your medical information kept confidential and disclosed only as permitted by law or with your authorization.
- To access your records. You have the right to review and obtain a copy of your medical records, and to request corrections to information you believe is inaccurate.
- To continuity of care. You have the right to information about continuing your care, including how to reach a clinician with follow-up questions.
- To voice a grievance. You have the right to raise a complaint about your care without fear of retaliation, and to receive a timely response.
- To transparent pricing. You have the right to know the cost of your treatment, including any recurring charges, before you are billed.
- To decline telehealth. You have the right to decline treatment via telehealth and seek in-person care instead.
Your Responsibilities
- Provide accurate and complete information about your health history, medications, and symptoms.
- Tell your clinician about any changes in your condition or any adverse reactions.
- Follow the treatment plan you agreed to, and ask questions when you do not understand something.
- Do not share your prescription medication with any other person.
