Telehealth Informed Consent
Last updated July 26, 2026
Please read before beginning a medical evaluation. Submitting an intake form means you consent to the terms below.
Telehealth Care
You consent to receive care via telehealth — evaluation and treatment by a licensed provider using secure electronic communication rather than an in-person visit.
Benefits & Limitations
Telehealth improves access and convenience. It also has limits: your provider cannot perform a physical examination, and some conditions cannot be diagnosed or safely treated remotely. Your provider may decline to prescribe and may recommend in-person care.
Risks
- Information you provide may be incomplete or inaccurate, affecting clinical judgment.
- Technology failures may delay evaluation or treatment.
- In rare cases a condition requiring in-person care may not be detected.
Medication Risks
Medications used for sexual health may interact with nitrates, alpha-blockers, and other drugs, and may not be appropriate with certain cardiovascular conditions. Disclose all medications and conditions during your intake. Seek immediate care for an erection lasting more than four hours, chest pain, or sudden vision or hearing changes.
Compounded Medications
Compounded medications are prepared by a licensed compounding pharmacy and are not FDA-approved. The FDA does not evaluate compounded products for safety or efficacy. Discuss risks and benefits with your provider.
No Guarantee of Outcome
No specific result is promised. Individual results vary.
Emergencies
Telehealth is not for emergencies. Call 911 or go to the nearest emergency department.
